MEDTEQ+ and AGE-WELL have unveiled 17 new collaborative projects through the envisAGE initiative. The selected projects address key priorities including aging in place, mobility, healthcare delivery, cognitive health, caregiver support and social inclusion. Together, partners from across Canada will help validate and accelerate innovative technologies designed to improve the quality of life of older adults.

Evaluation of the Implementation, Feasibility and Impact of Digital-Health Support for a Hybrid Older Adults Rehabilitation Program

Older adults often need rehabilitation after being admitted to the hospital for injuries or new symptoms from diseases. Rehabilitation aims to improve patients’ tasks so they can return home safely. When patients are discharged home, they often need more rehabilitation at home to continue improving their abilities and their health and wellness, and to prevent them from needing to return to hospital or be transferred to a supportive living center.

This project will test a new program at the Glenrose Rehabilitation Hospital that helps patients continue their recovery at home using the Tenzr Health Configurable Care Platform. The platform includes an app with which Clinicians can provide simple exercises, reminders, education, and virtual check‑ins. Optional sensors can help track movement and show patients how they are improving. Clinicians can watch progress from a distance and intervene early if something is going wrong.

Over two years, the team will implement and evaluate the usefulness of a new home rehabilitation program, delivered via the Tenzr App to improve patient function and to ultimately. determine if it can decrease the number of patients that need to return to the hospital. As part of this evaluation, the team will first determine the implementation feasibility of the App, including measuring the clinician and patient adoption, its suitable integration into clinical workflows and validation that it can be configured to deliver the right information to patients based on the program design.
